Data that will be gathered by the coordinators includes blood pressures and laboratories measured at clinic visits, number of clinic visits, hospital admissions, infections, rejections, graft loss, and number and type of medication errors. They will gather data via direct subject interview and through review of the subject’s electronic medical records.
