The Advancing Clinical Trials in Neonatal Opioid Withdrawal OBOE study is a multisite prospective longitudinal cohort study of infants with antenatal opioid exposure and unexposed infants from birth to 2 years of age (Clinical Trials.gov NCT04149509). The OBOE study protocol is described elsewhere in detail [19].

The study cohort consisted of all live infants born in Lombardy from January 1, 2016, to December 31, 2019. Lombardy is a region in Italy that accounts for approximately 16% of the country’s population, comprising nearly 10 million inhabitants.

A recent Lancet series highlighted the increasing global burden of mental health disorders, including maternal depression [17]. Although it is the most commonly diagnosed complication of childbirth, there are widespread gaps in screening, detection, and treatment of women affected by this incapacitating condition [18].

Records were obtained for the first pregnancy episode of all patients who received antenatal care at 8 academically affiliated practices and delivered at a large urban academic medical center from January 1, 2018, to July 22, 2021, in Chicago, Illinois. The study included all patients aged 18 years or older who attended at least 3 clinical encounters during pregnancy at the practices that used the EMR portal.

The timing of use was also raised, with some seeing value in repeating the process and starting sleep planning conversations during the antenatal period. They described seeing the value of the tool for all parents, not just those with infants at increased risk. Some suggested a variety of options for sharing the sleep plan image with the family, including printing it off for those without mobile devices.

The study also explores macro-, meso-, and microlevel contexts and implementation processes for improved quality and outcomes of maternal and neonatal health (MNH).
