One EHR system, Epic, allows for smart data elements (SDEs) to be triggered by built-in text or phrases that load preset text with components that can be edited by health care professionals [16,17]. This improves data entry efficiency and creates coded variables, from which data can be extracted in an automated way [18]. Each SDE element can have preset options or contain free text cells, in which providers can list multiple responses.

Epic Systems [6] (Epic Systems Corporation) was one of the early innovators in this space and is currently among the 5 leading EHR providers and the most adopted EHR worldwide [7,8]. In early 2023, Epic was independently rated as the best overall EHR suite for the 13th consecutive year and was in use in 89% of acute care hospitals in the United States [9]. Many academic and clinical organizations, such as Johns Hopkins University, have adopted Epic across their care delivery systems [10,11].

In 2014-2015, all physician documentation at Lee Health in Fort Myers, FL, was transitioning from a dictation-based system to a full EHR platform using Epic software (Epic Systems Corporation, Verona, WI, USA). The Epic platform was already integrated into obstetrical and neonatal nursing, respiratory therapy, case management, pharmacy, laboratory, and imaging data entry, and the physician or provider component was a mandatory next step. Dictation-based documentation was the documentation method of choice.
