Following source classification, the reviewers of this study subsequently evaluated the content of each article for quality using the Brief DISCERN score [23]. The cutoff score for this instrument was established as ≥16 out of 30 for a digital article to be considered good-quality content [23]. Following quality assessment, the text of each digital article was extracted onto a plain text document and evaluated for readability.

Figure 2 shows the DISCERN scores for all 87 websites across each item. Table 4 presents the mean score for each of the 16 DISCERN instrument criteria and the total DISCERN score. The mean total DISCERN score for all 87 websites was 32.6 (SD 5.5). Only 1 website scored more than 51 points, which is good; 9 websites scored between 39 and 50 points, which is fair; 72 websites were poor; and 5 websites were very poor.

After reviewing and discussing the DISCERN handbook, 2 authors (QW and LL) rated all the included websites. Interrater agreement was calculated. Overall mean and standard deviations for the items of the DISCERN quality criteria for the 96 included websitesa. a Each DISCERN item is rated on a 5-point scale, anchored at 1=the criterion was not met at all and 5=the criterion was fully met. Descriptive analyses provided a mean score for each website and a mean score for each DISCERN item.

The overall mean DISCERN score across all websites was 57.6 with a median score of 61 (IQR 49-68), a range of 18 to 80. The general DISCERN data properties are listed in Table 4, and the various scores are displayed in Figure 2. In total, 63% (36/57) of the websites had a DISCERN score of ≥52, a threshold that represents good-quality written materials. The 75th percentile of the total DISCERN score was ≥68, which was achieved by 28% (16/57) of the websites. These websites were considered high scoring.

The quality of the information in the videos was assessed using GQS, and the reliability was evaluated using DISCERN. The GQS consists of 5 criteria ranging from 1 to 5; a higher score indicates higher quality [26]. We used the first part of the DISCERN questionnaire to illustrate the reliability of a video—the higher the score was, the better was the reliability. The specific scoring details for GQS and DISCERN are shown in Table 1 and Table 2.

All raters read the PWQC manual (see Multimedia Appendix 1) and the DISCERN handbook and evaluated the websites independently of each other. Consumers were not included as the evaluations of content required an ability to critically appraise the clinical information. Statistical analysis was conducted using SPSS (version 24; IBM Corp). Each rater’s DISCERN and PWQC scores for each website were calculated as a mean score.

The two frequently used quality assessments DISCERN (name of the instrument) and Global Quality Scale (GQS) were applied to evaluate the medical quality of the posted videos [25,29-33]. DISCERN measures a video’s quality of information about treatment choices. It includes 16 questions addressing the reliability (questions 1-8), quality of health information (questions 9-15), and overall quality (question 16) of the videos.
